Add documents to a report you already filed

Records often arrive after the report does. This is the tool for those — the existing post-submission upload capability, kept alongside the new in-form upload rather than replaced by it (PWS 2.1, PRS#9).

This is the tool that is kept, not replaced

The redesigned form takes documents inside the reporting flow. This separate tool stays for records that only become available later — a discharge summary, a lab result, a vaccination record you had to request. PWS 2.1 requires both, and PRS#9 grades both.

1. Which report?

Enter the demonstration identifier shown on the confirmation screen when you submitted. It looks like DEMO-2600001.

2. Which documents?

Phase 1 file types. Phase 1 accepts medical and vaccine records only — PDF, Word, plain text, RTF, CSV and clinical exchange formats (HL7, CCD, C-CDA, FHIR JSON). Photographs and other images are the Phase 2 extension; the upload service already routes on file class, so enabling images is a configuration change rather than a rework.

This prototype records file names, sizes and types only. No file contents are transmitted or stored.
